Personal Questionnaire 

The Cayman Islands Monetary Authority requires that every Director, Officer, Manager and Shareholder who is a natural person, who wishes to be appointed to a company licensed under the Cooperative Societies Law (2001 Revision) or the Building Societies Law (2001 Revision), completes a Personal Questionnaire and submits it along with the supporting documentation described in the Cooperative and Building Societies Regulatory Requirements page.

Building Societies Form 

This form should be used for the submission of building societies' quarterly returns to CIMA. Quarterly returns must be submitted within 30 days of the end of the reporting period. 
 

Cooperative Socieites Form 

This form should be used for the submission of credit unions' quarterly returns to CIMA. Quarterly returns must be submitted within 30 days of the end of the reporting period.
