 Application - Class 'A' Local Insurer's Licence   

 Application - Class 'A' External Insurer's License   

This application form, duly completed and accompanied by the relevant supporting documentation and the prescribed fee, is used when applying to carry on insurance business as a Class "A" Insurer.

 Application - Class 'B' Insurer's Licence 

This application form, duly completed and accompanied by the relevant supporting documentation and the prescribed fee, is used when applying to carry on insurance business as a Class "B" Insurer.

 Application - Class 'C' Insurer's Licence 

 Application - Class 'D' Insurer's Licence 

 Application - Insurance Manager's Licence 
This application form, duly completed and accompanied by the relevant supporting documentation and the prescribed fee, is used when applying for a Licence to act as an Insurance Mananger within the Cayman Islands.

 Application - Agent/Agency Licence 

This application form, duly completed and accompanied by the relevant supporting documentation and the prescribed fee, is used when applying for a Licence to act as an Agent of an Insurance Company licensed to carry on business within the Cayman Islands.

 Application - Broker's Licence 

This application form, duly completed and accompanied by the relevant supporting documentation and the prescribed fee, is used when applying for a Licence to carry on business as an Insurance Broker within or from within the Cayman Islands.

 Personal Questionnaire
The Cayman Islands Monetary Authority requires that every Director, Officer, Manager and Shareholder who is a natural person holding more than 10%, who wish to be appointed to a company licensed under the Insurance Law (2004 Revision), complete a Personal Questionnaire and submit it with the supporting documentation described in the Insurance (Forms) (Amendment) (Licence Applications) Regulations 2003.
